Our interprofessional Hospital at Home concept supports hospitals and healthcare centers in establishing an innovative and patient-centered form of care. Together, we aim to shape the future of healthcare in Switzerland — sustainable, efficient, and compassionate.

Our Journey
The idea for hospitales emerged during the establishment of the Hospital at Home department at the Klinik Arlesheim in Switzerland, where it became clear that such initiatives come with numerous challenges. It quickly became apparent that we wanted to share our experiences in planning and implementing Hospital at Home with other interested healthcare institutions.
We were so enthusiastic about the concept that the vision of a nationwide, accessible Hospital at Home care system for all people in Switzerland was born. To move closer to this vision, we realized that a structured concept was needed — one that would prevent each institution from having to reinvent the wheel when setting up a Hospital at Home department and would enable a faster and more efficient implementation phase.
It wasn’t just us who believed in the vision of a nationwide Hospital at Home system; the Migros Pioneer Fund, part of the Migros Group's social engagement efforts, also strongly supported our initiative to create a support service for Hospital at Home implementation right from the start.
Thanks to this funding, the hospitales association was founded. In collaboration with the Arlesheim Clinic, we were able to translate our practical experiences into a comprehensive care concept — covering everything from planning a Hospital at Home department to practical aspects such as choosing the right visit backpack, medical equipment, process design, and fostering interprofessional collaboration.

Co-Founder and Board Member
Doctor of Medicine (MD) Severin Pöchtrager
Severin is a specialist in General Internal Medicine (FMH) and co-founder as well as a member of the management team at hospitales. As the senior physician in Internal Medicine and Head of the Hospital at Home department at the Klinik Arlesheim, he played a key role in establishing this department.
Severin is also a co-initiator and President of the Swiss Hospital at Home Society. In addition, he is a co-founder and part of the leadership team at the Arlesheim Academy for Anthroposophic Medicine.
His commitment extends to national and international speaking engagements on the topic of Hospital at Home. Severin is also a trained Loop Fellow for effective transformation support. His expertise and dedication make him a leading figure in the field of Hospital at Home care.
Co-Founder and Board Member
Doctor of Medicine (MD) Helene von Bremen
Helene von Bremen is a medical resident in Internal Medicine at KSBL Liestal and a co-founder of hospitales. She is also a board member of the Swiss Hospital at Home Society and is passionate about innovative team and organizational structures in medicine. As a Loop Fellow, she promotes forward-thinking care models that meet the needs of both patients and staff. She has gained experience at the Arlesheim Clinic in establishing the Hospital at Home department, as well as being part of the organizational team for "meine Station" at Aschaffenburg Hospital.
Her particular focus is on implementing brilliant concepts and experiencing their execution. Helene sees this as an opportunity to concentrate on essential patient care, reduce bureaucratic and social barriers in hospitals, increase self-efficacy in the workplace, and strengthen patient autonomy.

Co-Director
Isabel Misteli
She is a co-founder and co-managing director of hospitales. She played a key role in the founding of the Swiss Hospital at Home Society and successfully led the office during its establishment phase. Since 2023, Isabel has been managing the operations of the hospitales association and overseeing the secretariat of the Hospital at Home department at the Arlesheim Clinic.
With extensive experience in establishing and restructuring departments, as well as managing their administrative organization in hospitals, Isabel has gained valuable expertise. From 2014 to 2017, she worked in the administrative organization of Internal Medicine at the Kantonsspital Baselland, Bruderholz site. Between 2018 and 2022, she worked at Solothurner Spitäler AG at the Dornach site, specializing in administrative organization and project work in Internal Medicine.

Seraina Soldner
Seraina Soldner is an experienced project manager in the field of public health with a broad background in interdisciplinary team leadership, project analysis, development, and implementation. She has extensive knowledge in building partnerships in both the private and public sectors, as well as in public relations and fundraising. As Co-Managing Director at hospitales, she develops business strategies, oversees product development, and is responsible for the organization's public presence.
Previously, she managed projects at the Migros Pioneer Fund and co-founded SINGA in Switzerland, an entrepreneurship program for people with refugee or migration experience. Her academic background includes a Bachelor's and Master's degree in International Relations with a focus on EU law and international law, as well as a Master of Public Health, which she is currently completing. She is committed to a just and open society and aims to improve public health, particularly healthcare services, for the better.

Nurse Practitioner
Euryanthe Sprachta
Euryanthe Sprachta has extensive experience as a nurse practitioner in acute care, including intensive care, emergency care, and intermediate care. During the COVID-19 pandemic, she was part of a task force responsible for establishing a dedicated department for COVID-19 patients to effectively address the new healthcare challenges.
Since 2023, Euryanthe has been involved in developing the Hospital at Home program at the Arlesheim Clinic to establish this new form of acute medical care. In addition to providing holistic care for patients, she is also responsible for workplace and patient safety, onboarding new team members, and ensuring hygiene standards in home treatment settings. At hospitales, Euryanthe continues to develop the nursing concept for the Hospital at Home program and provides practical training sessions.
Physician
Doctor of Medicine (MD) Linn Thorsteinsen
Linn Thorsteinsen is a medical resident and has worked as a resident doctor, including at the Hospital at Home program of the Klinik Arlesheim. Through this experience, she witnessed firsthand how hospital-equivalent medical care can be successfully delivered at home and how active interprofessional collaboration can transform the approach to treatment.
During her medical studies in Zurich, she focused on patient-centered medicine. She initiated a tutorial for students and resident doctors, which also became the topic of her master's thesis. Currently, Linn is developing a treatment concept at hospitales to simplify the implementation of Hospital at Home in other institutions and to promote decentralized care.

The main reason for this is the solution-oriented approach. Employees are trained not to rely solely on the management level when facing problems but to find their own solutions."

Philipp Busche
Philipp Busche is the Chief Physician of Internal Medicine at the Klinik Arlesheim and also leads the Arlesheim Academy. He is a specialist in Internal Medicine and Gastroenterology with an additional qualification in Emergency Medicine. Furthermore, he is well-versed in anthroposophically extended medicine (VAOAS/GAÄD) and serves as the Executive Board Member of the GAÄD.
He completed his training at the Robert Bosch Hospital in Stuttgart and the Filderklinik in Filderstadt. His studies took him to the University of Rostock as well as the University of Tübingen.
Passionate about new working models and truly patient-centered medicine, he sees both excellently realized in the Hospital at Home project at the Arlesheim Clinic. His goal is to give this form of care both "wings" and "roots," making the concepts accessible to interested individuals and institutions while ensuring the care model matures in terms of medical and therapeutic effectiveness. He aims to inspire others with his enthusiasm and support its implementation.

Doctor of Medicine (MD) Tane Lammers
Tane Lammers is a medical resident in internal medicine and currently works at the Robert Bosch Hospital in Stuttgart. She was involved in establishing the Hospital at Home department at the Klinik Arlesheim and is a co-founder of the New Work Medicine e.V. association in Germany.
She completed her doctoral thesis in the field of innovative healthcare research at Goethe University Frankfurt, specializing in general medicine with a focus on developing local primary care centers.
For a long time, she has been exploring how the current challenges in healthcare can be addressed in the future. One possible solution she sees is the Hospital at Home concept, which enables patient-centered, holistic, and sustainable healthcare.
At hospitales, Tane is primarily involved in the scientific evaluation and further development of new forms of collaboration (New Work) in the context of Hospital at Home.
